TBCD Registration Forms
Registration packets for 2011 will be available in the spring of 2011.

You must include a signed release for each minor and adult and a medical release for each person registering.
|
|

Everyone attending TBCD is required by Baylor University to have an indemnity relase. This file has two releases, one for adults and one for minors. Please fill out one for each person attending TBCD. Do not forget to sign the releases.
